Guided Reading Log Sheet

This easy to use document has served me well in Kindergarten, 5th, and 6th grade classrooms. I keep a folder on each student and simply write down a few notes from our meeting together. It has a place for the date, book, and skill/s taught. (For older grades I also keep track of what page they are on. This allows me to see if they are truly using their time wisely and staying on task during our readers' workshop.)
