This sheet is the most touched paper in my entire classroom. During back to school night, each and every one of my students gets this Important Student Information Sheet filled out. Then every time that I need to contact a parent - for good or for bad - I pull out this sheet. I also make a note on a piece of notebook paper or on the back of the sheet itself about the contact so that I have a record of our communication.
