Back to School Freebie – Important Information Sheet

On the first day of school, I always send home a welcome letter about myself and this Important Student Information Sheet.  When students return the sheet, they get the first sticker for their sticker charts (a little bribery goes a long way) and I add the sheets to a three ring binder with some blank notebook paper behind it.  For the rest of the year, every time I want to send an e-mail, or make a phone call, I pull out that binder with everyone’s information.  On the notebook paper, I note down each contact I make with a parent, so that I can refer back to that communication if need be.  Download your free copy of this sheet from my Teachers Pay Teachers store.
