At the beginning of every school year, I like to collect information about my students. I like to make sure that I have on file information about how to contact anyone I need to in case of an emergency without having to contact the office. That’s why I use this form to help me with collecting student information.
Collecting Student Information in a Digital World
Most schools have the student’s information located online via a portal, the grade book, or some other electronic software. These come in handy, but I can tell you on many occasions I have been away from my desk or the system has been down. Sometimes it’s not always easy to be able to access that information immediately.
While most schools do like to collect the student information at the beginning of the year, I don’t always get access to that information in paper form. It usually gets stored in the student’s file or in the office.
I collect this form during open house and then place all my student’s information on a ring together. Then, I place it on my bulletin board next to my desk for when I need it right away. I always grab it immediately during a fire drill or a field trip. It comes in handy every time!
You can download this form yourself by heading over to my blog, The Owl Teacher, and downloading it yourself. It can be found on my post, Collecting Student Information. You can also find my behavior forms that are perfect for classroom management for free right here on Classroom Freebies!
Enjoy!
