Need an easy way to keep track of all that important information about your students? I give this Important Student Information Sheet to my parents at the beginning of each school year. Then, I take the completed sheets and put them in a binder all together. Behind each sheet, I leave a blank page. On that page, I keep track of each time I contact a parent, through phone or e-mail with simple anecdotal notes. It’s a great, easy way to keep up with parent communication during the year. I make a similar sheet each year and keep it by my phone. I like that you included the sibling list. It is really nice to know this information when the child in your class is sick and you want to send work home.
