Student Behavior Forms: Stop the Crazies!

Learn how to calm down student behavior with these free student behavior forms.  Easy classroom management tool to encourage students to reflect on their behavior by completing a Student Behavior Report.

This is the time of year that our little darlings start going a little crazy in the classroom.  With Spring Fever (yes, it is a real thing at school) and, dare I say, the end of the school coming up faster than we’ll realize; we all need something to help keep our students on track with learning.

This Student Behavior Report is a tried and true resource, and much used classroom management tool.  It is so simple to implement, plus you can very easily use it right along with your classroom rules, visual behavior chart, daily procedures, etc. that you already have in place.

Whether you teach 1st, 2nd, 3rd, 4th, 5th, 6th, etc. you can make this documentation form work for your students.

I love it because the students are actually doing the thinking, writing, and documenting.  Your students will fill it out themselves.  

What does the Behavior Form ask?

  1. Write three complete sentences that explain your unacceptable behavior.
  2. Write three complete sentences that explain how you will change your behavior and make better choices.

Of course you’ll notice that I threw in “complete sentences” into the questions.  I love to give my students any opportunity to practice writing, especially complete sentences.  Typically, if the student turns the Behavior Report in without it being thorough and written in complete sentences, I will hand it back to them.
